At first, the anatomical essentials of Sepsis and septic shock are discussed which are needed to be crossed by microbes to cause the Infection and if it reaches the blood then systemic Inflammation leads to sepsis and then to septic shock. Following this, elaboration is given on physiology of systemic oxygen transport. After that, cardiovascular insufficiency and global tissue hypoxia is delineated.
Risk factors of severe Sepsis and mortality are concomitant medical diseases, diminished cardiopulmonary reserve, and age-related decrease in organ function. Then physical examination is carried out by checking the temperature, heart rate, and shock index.
Likewise, light is shed on details of variety of variables deliberated in diagnostic testing. These variables encompass general variables, inflammatory and hematologic variables, organ dysfunction variables, hemodynamic variables, and tissue perfusion variables.
Then ED management of an adult patient in first 6 hours and subsequent 24 hours is pursued. Moreover, stages of Sepsis and therapies are elaborated accordingly. Volume therapy is also discussed along with intensive insulin therapy. Another subject of discussion in this lecture is the diagnosis of Paediatrics sepsis.
